Notice
JD-HM-7 — Notice to Quit (End) Possession
Superior Court — Housing Session (Summary Process) · revised 4-19 · fillable PDF
The quit date must leave at least three full days between service and that date (C.G.S. 47a-23); we only fill it in if you typed the notice deadline. A state marshal must serve the notice; the Return of Service is completed by the marshal.

JD-HM-8 — Summary Process (Eviction) Complaint – Nonpayment of Rent
Superior Court — Housing Session · revised 2-19 · fillable PDF
Tick the Judicial District or Housing Session and write its town yourself (see the court locator on jud.ct.gov), and choose the return date (any weekday except a legal holiday, as the summons instructions explain). Paragraph 4 needs the date the unpaid rent was due; write it in if it is blank. Attach the Notice to Quit (JD-HM-7).
